Pre-elementary

  1. Nursery
    1. The child must be four years old by August
    2. Photocopy of NSO, and baptismal certificates
    3. Two copies of 1x1 colored ID pictures
    4. Admission test and interview
  2. Kindergarten
    1. The child must be five years old by August
    2. Photocopy of NSO, and baptismal certificates (for new students)
    3. Two copies if 1 x 1 ID pictures (colored)
    4. Admission test and interview.

Elementary

  1. New Students (Grade 1-6)
    1. Grade 1 applicants must be six years old by August
    2. Report card (Form 138)
    3. Photocopy of NSO, and baptismal certificates
    4. Two copies of 1x1 colored ID pictures
    5. Letter of Recommendation from the principal or guidance couselor (original copy)
  2. Old Students (Grade 1-6)
    1. Report card (Form 138)
    2. Satisfactory grade in ACADEMICS and CONDUCT otherwise, the student is placed under academic or disciplinary probation
    3. No academic deficiency

Junior High

  1. New Students (Grade 7-10)
    1. Grade 1 applicants must be twelve years old by August
    2. Report card (Form 138)
    3. Photocopy of NSO, and baptismal certificates
    4. Two copies of 1x1 colored ID pictures
    5. Letter of Recommendation from the principal or guidance couselor (original copy)
  2. Old Students (Grade 7-10)
    1. Report card (Form 138)
    2. Satisfactory grade in ACADEMICS and CONDUCT otherwise, the student is placed under academic or disciplinary probation
    3. No academic deficiency

Night High

  1. New Students
    1. Report card (Form 138)
    2. Certificate of Good Moral Character (original copy)
    3. NSO Birth Certificate (original and photocopy)
    4. Baptismal Certificate (photocopy)
    5. Income Tax Return (of both parents)
    6. Two copies of 1x1 ID pictures (colored)
    7. Sketch of Residence (with landmarks and important information)

Senior High

  1. New Students (Grade 11-12)
    1. Report card (Form 138)
    2. No academic deficiency
    3. National Career Assessment Examination (NCAE) Certificate of Rating (COR)
    4. NSO Birth Certificate (original and photocopy)
    5. Baptismal Certificate (photocopy)
    6. Two copies of 1x1 ID pictures (colored)
    7. Certificate of Good Moral Character (original copy)
    8. Admission test and interview

Scholarship

The CIC Upper Basic Education Department offers the following

Full and half scholarship to the valedictorian and salutatorian respectively, from an elementary school recognized by the governament with the following criteria:

  1. at least 150 graduates from a public school
  2. at least 50 graduates from a private school
  3. Proper certification from the Principal
  4. Very satisfactory grade of the test administered by the CIC Counseling and Testing Services

Scholarship covers only tuition fees.
